Care Coordination Services - 154Syracuse, NYClinicalFull-Time, Days, 8:00am-4:30pm w/ occasional weekendsReq # 20792Posted 10/31/2019 The Social Worker works collaboratively with the Care Managers and other members of the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care. The Social Worker participates in the development and implementation of a comprehensive integrated discharge plan. The Social Worker identifies patient and family psychosocial and environmental needs to admission, diagnosis, treatment and discharge. The Social Worker actively participates in the discharge process following federal and state regulations and accreditation standards managing the care across the continuum to ensure a safe discharge within a timely manner. The Social Worker provides crisis interventions to patients and families as needed.
Job Requirements: MINIMUM EDUCATION / CERTIFICATION / LICENSURE Required: Masters Degree in Social Work. Preferred: LMSW licensing preferred EXPERIENCE Required: 2 – 3 years Social Work experience. Preferred: Case Management, Discharge Planning or home care experience preferred. SPECIAL SKILLS/EQUIPMENT Excellent oral and written communication and interpersonal skills. Demonstrate age appropriate communication in assigned work area. Ability to interact with physicians, members of the multidisciplinary healthcare team, patients and families and community agencies to achieve optimal patient outcomes. Knowledge of federal and state regulations (DOH, Medicare/Medicare). Strong organizational and problem solving skills. Knowledge of third party payers and /or managed care principles. Additional Info prefer experience in Maternal Health, including but not limited to post-partum depression, community resources, family dynamics and navigating the NICU environment.
